Integration of Physical and Behavioral Health Care

Our Clinic offers integrated physical and behavioral health care.  The relationship between physical and psychological well-being is complex, particularly among older adults who may have chronic medical conditions and take multiple medications.  The integrated approach results in overall improvement of health and reduced healthcare costs.

The psychiatrists, social workers, and psychiatric nurse practitioner at SPOP collaborate on client care, which can include:

  • Physical and mental health screenings
  • Individual and group therapy
  • Specialized substance use counseling
  • Monitoring of chronic medical conditions
  • Education in the interplay between physical and psychiatric illnesses
  • Medication management
  • Support for smoking cessation
  • Coordination of treatment with medical providers
  • Assistance in making medical or dental appointments
  • Counseling in diet, nutrition, and weight loss
  • Health and Wellness education
